That’s a rather specific request, and after the overhaul of the terrain generator code in the 1.8 release its unlikely that we could find you such a seed. My advice would be to try a world editor such as MCEDIT (Windows/Mac/Linux compatible) to create a flat area at the bedrock level.

Virtually any seed will do for that. If you don’t like the epic mountain-scapes in some seeds, wander off and you’ll find a quaint enough forrest in most. That is the beauty of Minecraft.

start next to a spawner and a crate with wheat, saddle, gunpowder, string. and a huge cave with lots of coal and iron, and a massive lava fall!!!

Commenting on your Bonus Tip from not too long ago, an alternative to using a nether portal would be using Sugar Cane to make a door. Granted, it would need a pool of water somewhere next to it when it is isolated and made underwater, buy hey-- you're surrounded by the stuff. Besides, it doesn't make any noise that an unwanted miner might notice, and it is usually a dark or earthy color so it is harder to notice underwater.
I’ve heard of that as well. I always have troubles with it, though, and don’t like having to put a pool of water nearby. But it definitely is valid.
In terms of the noise of a nether portal, Notch just recently nerfed the distance away from a nether portal where the sound can still be heard.
